HOMEMAKER SERVICES

Homemaker Services refer to the direct care that a client receives. This includes the following:

• Bathing, Dressing, Grooming
  
• Essential Errands/Grocery Shopping
  
• Assistance with Environmental Tasks
  
• Meal Preparation
  
• Transportation
  
• Assistance with Community Activities.

Please note: Hours of service are based on the client's Level of Care which is determined by the Medical Eligibility Form.

This form is completed by a nurse from West Virginia Medical Institute (WVMI).

Hours of service range from 62-155 hours per month.
